About this check

This site answers one question: is this a real, licensed, enrolled provider at this address? It reads only free government records. It never decides whether a trip, visit or service is covered.

It collects no names, member IDs, dates of birth or trip details. Every search is logged with your account, IP address, the address searched and the result, for audit.

“No match” means “not found in public sources”, not “not a real provider”.
